The Henry Kaufmann Campgrounds is pleased that you have chosen to return to us for the summer of 2012.

Please read the following information on how to ensure that you are hired for the summer.

Personnel Forms:

Before we will be able to confirm your employment we will need to receive from you the following documents:

Notification of Intent to Return

• to work in Staten Island
• to work in Pearl River
• to work in Long Island

Application Update for Returning Employees
Authorization to Release Information

Please Note: HKC will be conducting state mandated background checks on all new and returning employees

Additional Forms:

W-4: You must submit a new W-4 form for 2012
Working papers: If you are a minor, you may need to submit updated working papers. If you are unsure, please check with your school.

Certifications:

You may need to be recertified this year. Please check your certifications. If you are unsure, please contact our office.

Lifeguards:
Current American Red Cross Lifeguard Certificate
Current American Red Cross AED/CPR Certificate

RN/LPN/EMTs:
Current RN, LPN or EMT certificate
Current American Red Cross AED/CPR Certificate

Maintenance:
Current Driver’s License

Please note: the Henry Kaufmann Campgrounds will not be able to rehire you until all required documents are received.

All forms will need to be printed out and signed.
